Letter of Intent Form


  • The number of topics you are applying to offer (a minimum of 2 topics, each will be repeated).
  • Please include anatomy site and technique of the topic (e.g. Oral fixation for SRS).
  • Describe the format of the presentations for the topics submitted (e.g. hands-on workshop or lecture/demonstration).
  • Each workshop has a maximum of 30 attendees.
  • Each lecture/demonstrations has a maximum of 100 attendees.
